ABOUT THE PROGRAM
The Associated’s Student to Student is a peer-education program (the “Program”) in which Jewish high school student volunteers (“Participants”) travel to public and private schools in the greater Baltimore area to give classroom presentations about Jewish identity, Jewish life and traditions, Israel, and antisemitism. Presentations take place on the premises of host schools and other third-party locations that are not owned, operated, or controlled by The Associated, and involve open classroom discussion and audience questions, and may touch on personal and sometimes sensitive topics.

PARTICIPANT CONDUCT
In return for permission from The Associated for my child to be a Participant in the Program, as the parent or guardian or the Participant, I acknowledge and agree to the following:
I consent for my child to be a Participant in The Associated’s Program. I understand that Participants are expected to follow Program guidelines and the reasonable instructions of Associated staff and volunteer chaperones at all times, including while at host schools and during transportation. The Associated may, at its discretion, remove a Participant from a presentation or from the Program for conduct that is unsafe, disruptive, or inconsistent with Program guidelines.

I consent for my child to be a Participant in The Associated’s Program. TRANSPORTATION AUTHORIZATION OPTIONS

In order to ensure that Participants have appropriate transportation, please select all transportation methods that you authorize for your child. Transportation will only be provided using methods authorized below.
Parent Arranged Transportation

Parent Arranged Transportation I can transport my child to and from program presentations, meetings, and activities.
Transportation by The Associated
All Associated staff and volunteer drivers who transport Program participants will have completed a background check and driver-eligibility review in accordance with The Associated's volunteer driver policy. The Associated’s practice is that participants will not be transported one-on-one with an adult. There will always be a minimum of three people in the vehicle: one adult and at least two participants.

Transportation by The Associated
All Associated staff and volunteer drivers who transport Program participants will have completed a background check and driver-eligibility review in accordance with The Associated's volunteer driver policy. The Associated’s practice is that participants will not be transported one-on-one with an adult. There will always be a minimum of three people in the vehicle: one adult and at least two participants. I authorize my child to be transported by employees of The Associated and approved volunteers of The Associated, including parent volunteers.
Self-Transportation by Participant

Self-Transportation by Participant I authorize my child to drive themselves to and from program presentations, meetings, and activities. I do not authorize my child to drive themselves.
Participants are not permitted to transport other student participants to or from program presentations, meetings, activities, or events. If Self-Transportation is selected, I agree to the following terms:

Transportation undertaken in my child's personal vehicle is at my and my child's own risk. My child will wear a seatbelt at all times while driving and will not use a cell phone or other mobile electronic device to send text messages while driving. My child will drive directly to and from the program location, will not make unauthorized stops, and will not transport any other Program participant, consistent with the Student Driver Restrictions above. I will provide The Associated with a copy of my child's driver's license and proof of insurance upon request, and this self-transportation authorization is void at any time that documentation is not current or is not provided.
ASSUMPTION OF RISK
I understand that participation in the Program, including presentations, related meetings, trainings, and travel to and from schools and other program locations, involves inherent risks, including but not limited to:

• Personal injury, illness, or property damage while at a host school or other program location not owned or controlled by The Associated
• The conduct, supervision practices, and physical condition of premises maintained by host schools and other third parties, over which The Associated has no control
• Being photographed, recorded, or quoted by school staff, students, or media during a presentation
• Risks associated with public speaking and with disclosing personal, family, or religious information to an audience of peers
• Automobile accidents
• Personal injury
• Property damage
• Illness or medical emergencies
• Road, traffic, or weather conditions
• Mechanical failures
• Actions or omissions of third parties.
I voluntarily permit my child to participate in the Program and voluntarily assume all risks associated with that participation, including transportation to and from the Program, whether arising from the negligence of the Associated Persons (as defined below) or otherwise, except as set forth under “Limitations on Release” below.

RELEASE, WAIVER, COVENANT NOT TO SUE, AND INDEMNIFICATION
To the fullest extent permitted by law, I, on behalf of myself, my child, and our respective heirs, executors, administrators, and assigns:

Agree to indemnify and hold harmless the Associated Persons from any loss, liability, damage, or cost, including reasonable attorneys' fees, that the Associated Persons may incur arising out of my child's participation in the Program, to the extent permitted by law.
NO RESPONSIBILITY FOR THIRD-PARTY PREMISES AND PERSONS

I understand that presentations and related activities occur on the premises of host schools and other third parties that are not owned, operated, staffed, or controlled by The Associated. The Associated is not responsible for the condition of those premises, the supervision or conduct of host school staff or students, or the acts or omissions of any other third party.
NO INSURANCE PROVIDED

NO INSURANCE PROVIDED I understand that The Associated does not carry or maintain health, medical, dental, or disability insurance coverage for Program participants, and that I am solely responsible for my child's own medical and health insurance coverage. Any medical expenses incurred in connection with my child's participation are my responsibility.
MEDICAL EMERGENCY AUTHORIZATION

MEDICAL EMERGENCY AUTHORIZATION In the event of a medical emergency during transportation or participation in program activities, I authorize The Associated and its representatives to obtain emergency medical treatment for my child if I cannot be reached immediately. I have disclosed above any severe allergies or medical conditions I want adult supervisors to be aware of, and I understand The Associated is relying on that disclosure. I understand that any medical expenses incurred are my responsibility.
PHOTOGRAPHY AND MEDIA

PHOTOGRAPHY AND MEDIA I understand that Program presentations may be observed, photographed, or written about by host school staff, students, or local media, and that The Associated may photograph or record Program activities for internal, educational, or promotional purposes.
